Job summary

Simaril Inc. supports individuals with intellectual and developmental disabilities in a community setting. The Outreach Support Mentor provides person-centered support, helps individuals achieve their goals, and collaborates with the team to meet support plans.

You will work on-site for 2 days a week (Tuesday and Wednesday), with shifts from 3:00 PM to 11:00 PM, providing coaching, prompting, and assistance while ensuring safety and dignity in daily activities and household tasks.

Qualifications

• Proof of legal eligibility to work in Canada required.
• 2 years' experience working with adults with an intellectual disability.
• Knowledge of disabilities, disorders, and mental health issues.
• Excellent communication skills and advocacy ability.

Responsibilities

• Provide person-centered support to individuals based on their needs, abilities, preferences and goals.
• Assist, prompt, model, coach, guide and encourage daily living tasks and activities.
• Manage medications following proper protocols and documentation.

The Role

As the Outreach Support Mentor, you will provide person-centered support to individuals based on their individual needs, abilities, preferences, and goals. Staff are available on-site 8 hours a day and provide support through assistance, prompting, modelling, coaching, guidance, encouragement, and skill development while respecting each person’s abilities, choices, preferences, pace, and goals.

Outreach Support Mentors collaborate closely with other team members to ensure that the needs outlined in support plans are met and to empower individuals to achieve their goals.

How You'll Make a Difference

• Provide a Person-Centred Support
• Provide respectful, individualized support based on each person’s needs, preferences, abilities, and goals recognizing that these may change from day to day or throughout a shift.
• Break larger goals into manageable steps and provide appropriate coaching, modelling, prompting, and encouragement.
  
• Provide support for the day to day activities outside the cluster home.
• Encourage individuals to take an active role in household activities and decisions.
• Attend leadership meetings.
• Managing medication regimens, following proper protocols and documentation.
• Meal preparation and cooking with consideration on dietary needs and preferences.
• Maintaining clear and accurate documentation including incident reports, communication logs and daily notes.
  

Your working hours will be:

• Tuesday: 3:00PM – 11:00PM
• Wednesday: 3:00PM – 11:00PM

What You Need to be Successful

• Proof of legal eligibility to work in Canada will be required as part of the recruitment and onboarding process
• Post-secondary education in Community Support Work, Disability and Community Support, Social Services, Mental Health, or a related field is considered an asset
• 2 years' experience working with adults with an intellectual disability
• Knowledge of disabilities, disorders, and mental health issues
• Excellent communication skills
• Advocacy skills
• Flexibility

Other Important Requirements

• Valid Manitoba Driver's License with a satisfactory driver's abstract is required.
• Use of a safe vehicle for work purposes
• Current Food Handler Certificate is required.
• Criminal Record Check with Vulnerable Sector Search
• Adult Abuse Registry Check
• Child Abuse Registry Check
• Current CPR and First Aid certification
• Experience providing personal care is an asset

Preference May Be Given To Candidates Who Have

• Experience working with mental health

• Experience working within the justice system
• Experience supporting individuals with co-occurring disabilities
