The Salvation Army seeks an Outreach Case Manager to provide support at their Halton Lighthouse location. Job Description:
Working hours: Tuesday to Saturday 9:00 am to 5:00 pm
Supervision: Program Coordinator or designate.
Salary: $20.97 hourly
Union: SEIU Local 1
Location: Halton Lighthouse, The Salvation Army
Status: Permanent Full Time
Service Responsibilities:
The incumbent is responsible for the following range of job duties characteristic, or typical of the job class:
1. Provides outreach case management support to programs at Lighthouse shelter including but not limited to clients in hotel and in the broader Halton area.
2. Monitors environment and clients/residents to ensure safety and security; completes regular client/resident status checks and room searches as required
3. Develops and implements case management of assigned clients/ residents; assists clients/residents in establishing goals, monitoring progress, and prepares discharge plan 4. Maintains and updates all appropriate documentation related to clients/residents; initiates any new documentation; completes all reports required by the funder and The Salvation Army
4. Provides safe storage and recording of prescribed medication Coordinates and facilitates resident programming as required
5. Conducts all required shift change procedures prior to and after shift change Performs intake and discharge procedures for clients/residents as assigned
6. Monitors appropriate behavior, intervenes in crisis situations, provides emotional support and advocacy
7. Distributes funds to residents in accordance with established guidelines
8. Engages in the development of inter-personal relationships that promote dignity and respect
9. Performs check-in of residents on passes in community; escorts residents and monitors behavior
10. Liaises with Police, Funders, court personnel, community agencies, families and other staff members
11. Provides orientation to newly hired staff, volunteers, students Provides back-up support to front line positions and their functions as required
12. May be required to attend resident appointments
13. Attend and facilitate case conferences with case management team
14. Collect and record case management statistics daily
15. Prepare monthly statistical and outcome measures reports for Director
16. Review and forward weekly update reports to the Region of Halton
17. Review all resident intakes, discharges and restrictions
18. Prepare weekly Personal Needs Allowance requisitions
19. Attend all seminars and training sessions as requested by Management
20. Assist in any other duties or responsibilities as requested by Management
21. Adhere to standards based upon values and philosophy of The Salvation Army and the Halton Lighthouse Participate in all Case Management and Staff Team Meetings Knowledgeable of Fire Safety and Emergency Evacuation Plans Responsible to work safely and report unsafe or unhealthy working conditions
22. Engage in formal evaluation sessions with his/her supervisor

The performance of the job requires attentive listening and observation of body language, continuously monitoring the environment, multitasking, and reading.

The incumbent is involved in minor financial matters regularly, or on a relief basis, or has responsibility for the wise use of his/her own resources. He/She may be authorized to spend, disburse, or collect small amounts of cash. The incumbent has very little input into the annual budget.

Compensation:
The target hiring range for this position is $20.97 to $21.45 with a maximum of $21.92.Placement in the salary range will be based on factors such as market conditions, internal equity, and candidate experience, skills, and qualifications relevant to the role.
