Job summary

Alberta Health Services is seeking a Social Worker II to join the Psychosocial Oncology team at Central Alberta Cancer Centre. You will provide psychosocial assessment, counselling, case management and discharge planning to cancer patients and families in an outpatient setting.

You will collaborate with social workers, psychologists, spiritual care and other navigators, with initial consultations by phone and follow-up as needed.

Qualifications

• Bachelor's degree in Social Work completed.
• Active or eligible for registration with the Alberta College of Social Workers (ACSW).
• Minimum 1 year oncology setting experience.
• Minimum 2 years medical setting experience (acute/ambulatory/community).
• CPR (BCLS) and ergonomic/ergonomics prerequisites.
• Experience with PFAP and financial assistance requests.
• Ability to work in a fast-paced outpatient clinic with diverse patients.

Responsibilities

• Conduct psychosocial assessments and resource-based interventions.
• Provide individual, group and family treatment and counselling.
• Perform risk screening and case management/coordination.
• Assist with transition or discharge planning and public awareness activities.
