Job summary

Sunnybrook Health Sciences Centre is seeking an experienced Social Worker to provide advanced level psychosocial services within Odette’s Patient & Family Support Program for breast cancer patients. You will conduct psychosocial assessments and lead Goals of Care discussions, offering counselling and emotional support in an interprofessional team.

Qualifications

• Masters Degree in Social Work required.
• Must be a current member in good standing with the OCSWSSW.
• Minimum 5 years’ previous experience working in a health care setting preferred.
• Demonstrated Psychosocial Oncology knowledge preferred.
• Psychosocial provision of care to those with breast cancer preferred.
• Proven excellent assessment, counselling and problem solving skills essential.
• Superior communication, negotiation, mediation and collaborative interprofessional team skills required.
• Well-established knowledge of relevant health care system processes and community resources required.
• Superior time management, organization, and prioritization skills in addressing multiple tasks and timelines in a fast-paced environment required.
• Proven commitment to person and family-centred care essential.
• Demonstrated strong commitment to academic practice required
• Student education required
