Qualifications

• Licensed and registered with the Ontario College of Pharmacists.
• Previous experience in hospital pharmacy.
• Retail pharmacy experience in oncology setting preferred.
• Demonstrated ability to exercise independent judgment.

Responsibilities

• Provide clinical care to oncology patients and monitor medications.
• Manage injectable anti-cancer and take-home oral regimens.
• Maintain electronic patient records and conduct drug regimen reviews.
• Provide cross-site coverage at Centenary outpatient clinic pharmacy.
