Peterborough Regional Health Centre (PRHC) is seeking an Occupational Therapist to support the Peterborough Homelessness Addiction and Recovery Team (HART) Hub. The OT will work with clients presenting with mental health and/or substance use concerns, providing assessment, rehabilitation, and case management within an integrated health team.

Requirements include a B.Sc. in Occupational Therapy and registration with COTO, plus experience with homelessness and crisis intervention.

Qualifications

  • B.Sc. in Occupational Therapy or equivalent.
  • Registered with the College of Occupational Therapists of Ontario (COTO).
  • Training in mental health and addiction models of intervention.
  • Experience working with individuals who are experiencing homelessness.
  • Excellent assessment, risk, and crisis intervention skills.

Responsibilities

  • Provide client-centred occupational therapy assessments and interventions for individuals with mental health and addiction challenges.
  • Collaborate with the Integrated Health Team to deliver continuum of care.
  • Support housing with supports and systems navigation through the HART Hub.
  • Adapt interventions for community settings and manage risk in high-stress situations.

The Peterborough HART Hub is designed as a low-barrier, trauma-informed, culturally responsive, health care model that integrates primary care, mental health, substance use treatment, housing with supports, and health systems navigation. The Registered Nurse (RN) would be responsible for delivering services effectively and in a way that meets the needs of the individuals they serve.The Occupational Therapist (OT) will work with clients presenting with significant mental health and/or substance use concerns using a client-centred, evidence-based service model based on standards of best practice.The OT will collaboratively provide a continuum of client-centred assessment, treatment, rehabilitation, recovery interventions, and case management services for individuals in the community with mental health and addiction challenges and are experiencing homelessness. The purpose being to promote recovery, prevent relapse and to enable clients to improve their quality of life. The Occupational Therapist is an integrated role on the health care team at the Peterborough Homeless and Addiction Recovery Treatment (HART) Hub. Integration on this team means working collectively to provide team-based care that is collaborative, comprehensive and client centred.Jointly reporting to the PRHC Manager, Adult Outpatient Mental Health & Addiction Programs, and the Fourcast Addiction Services HART Hub Manager, the OT will work onsite at the Peterborough HART Hub as part of the Integrated Health Team. Through daily onsite work, the members of the Integrated Health Team will report to the HART Hub manager. Through a Memorandum of Understanding (MOU) between PRHC and Fourcast, the OT will report daily to the HART Hub manager with shared oversight of the role between the PRHC and Fourcast managers. The PRHC and HART Hub managers each hold distinct but complementary areas of oversight, with shared responsibility for collaboration, performance outcomes and service quality.WHAT YOU’LL BRING:B.Sc. in Occupational Therapy or equivalent.Registered with the College of Occupational Therapists of Ontario (COTO).Training and/or education in mental health and addiction models of intervention.WHO YOU ARE:Experience working with individuals who are experiencing homelessness.Demonstrated expertise in providing clinical services to persons with mental health and/or addictions.Well-developed skills for assessing individual functionality and recommendations for appropriate supported and/or independent living environments.Excellent assessment skills and ability to adapt to community settings.Well-developed knowledge of recovery principles for addiction and mental illness.Excellent skills for assessing mental status, and risk.Excellent ability to plan and implement individual and group programs.Comprehensive knowledge of mental health related disorders and treatment approaches, as well as an ability to work with individuals dealing with serious mental illness.Excellent knowledge of community services and ability to advocate.Comprehensive working knowledge of relevant legislation.Excellent problem-solving skills and ability to make decisions in stressful, sometimes high-risk situations.Excellent crisis intervention and de-escalation skills in community settings, without backup.Ability to manage stress, time and to work independently.Valid driver’s license, vehicle in good repair and required insurance coverage.Willing to transport clients in own vehicle.Three years’ related experience preferred.This position is for a current vacancy.OUR COMMITMENT:At PRHC, we take great pride in maintaining an inclusive culture of respect and a diverse workforce that reflects the community we serve.
