Job summary

Baycrest Hospital is seeking an Occupational Therapist to join its interdisciplinary geriatric rehabilitation team. The role provides OT assessment and treatment across Complex Continuing Care, Slow Stream, Musculoskeletal Rehab High-Tolerance areas and Behavioral Neurology, with potential reassignment based on clinical needs.

Qualifications

• Licensed Occupational Therapist in Ontario.
• Experience with older adults with multiple medical conditions and/or cognitive impairment and mental health issues.
• Excellent interpersonal, communication, and problem-solving skills.
• Proficiency with Meditech, MDS, Word and Excel; NRS/FIM preferred.
• ADP authorizer preferred.

Responsibilities

• Conduct client assessments, treatment planning and intervention for older adults in complex continuing care settings (direct and consultative models).
• Participate in clinical decision-making and advocate for clients with cognitive, medical or mental health changes.
• Assist with discharge planning in complex cases and coordinate with care teams.
• Supervise or delegate tasks to OTA/PTA when appropriate.
• Contribute to teaching programs for OT and OTA/PTA students.
• Support evidence-based practice and program evaluation in collaboration with the inter-professional team.

Job Summary

Be part of Baycrest’s interdisciplinary specialized geriatric rehabilitation team. This role will provide occupational therapy assessment and treatment services to the Complex Continuing Care unit, Slow Stream Rehabilitation, Musculoskeletal Rehabilitation High-Tolerance areas, Behavioral Neurology and as clinical operational needs require, may be re-assigned to other areas. Reporting directly to the Manager and Interprofessional Practice Chief for Occupational Therapy, Physiotherapy & Therapeutic Recreation, the occupational therapist will also communicate and collaborate with Clinical Unit Managers to facilitate optimum patient care delivery.

Key Responsibilities

• Conducting client assessments, treatment planning and intervention as well as some discharge planning for older adults in a complex continuing care environment, utilizing both direct and consultative models of service delivery.
• Playing a key role in clinical decision-making and client advocacy for clients who may have multiple medical &/or mental health issues, and be experiencing changes in cognition, deconditioning, neurological and/or musculoskeletal conditions.
• Involvement in multi-faceted discharge scenarios and working with complex support systems
• Comfort assigning to OTA/PTA when appropriate.
• Participating actively in the teaching programs for occupational therapy and OTA/PTA students.
• Supporting a systematic, evidence-based approach to client care, program development and evaluation, and professional practice, in close collaboration with inter-professional team members, volunteers and the Occupational Therapy clinical team.

Qualifications

• Licensure in good standing with the College of Occupational Therapists of Ontario
• Confirmed status as ADP authorizer preferred
• Experience with an older adult population with multiple medical conditions and/or cognitive impairment as well as mental health issues.
• Excellent interpersonal, communication, and problem-solving skills
• Basic computer skills – Meditech, MDS, Microsoft Word and Excel, NRS/FIM preferred.
