Job summary

Bayshore HealthCare in Toronto seeks a passionate Occupational Therapist for our Transitional Care Units. This full-time position requires day shifts from Monday to Friday, focusing on patient assessments, treatment planning, and collaboration with health team members.

Job Overview

Bayshore HealthCare seeks a passionate and caring Occupational Therapist for our Transitional Care Units in the Greater Toronto Area. Full‑time position with day shifts Monday to Friday.

Responsibilities

• Conduct assessments and establish, evaluate, and revise treatment programs, including goals and discharge plans.
• Maintain accurate clinical records in accordance with College of Occupational Therapists of Ontario standards.
• Document assessment results, goals, treatment progress, and discharge plans.
• Collaborate with health team members.
• Communicate pertinent information and recommendations to clients and care providers, and provide education on treatment strategies.
• Observe and guide others involved in the OT program.
• Maintain active standing with the College of Occupational Therapists.
• Participate in ongoing internal and external education activities.
• Adhere to Bayshore policies and procedures.
• Engage in quality activities and continuous improvement initiatives.
• Participate in health and safety activities; notify the supervisor of any risks.
• Maintain confidentiality of client and corporate information.
• Complete other tasks as requested.

Qualifications

• Master’s Degree or recognized equivalent in Occupational Therapy.
• Registered and in good standing with the College of Occupational Therapists of Ontario.
• Preference for 3+ years of OT service experience.
• Experience with clinical patient assessments and treatments; knowledge of medical/legal report writing is an asset.
