Job summary

Bayshore HealthCare is seeking an Occupational Therapist to provide in-home assessment, treatment, education, and case management under supervision. The role requires Masters in Occupational Therapy and active registration with the Ontario College of OT, with a preference for three years of OT experience and a valid driver's license.

The OT will maintain clinical records, collaborate with the health team, and educate clients and care providers on strategies.

Qualifications

  • Masters Degree in Occupational Therapy required.
  • Registered and in good standing with Ontario College of Occupational Therapists.
  • Three years' experience preferred in delivering occupational therapy services.

Responsibilities

  • Assess clients and establish/revise treatment programs with goals and discharge plans.
  • Maintain accurate clinical records per College standards; document results, goals, progress and discharge plans.
  • Collaborate with health team; communicate recommendations and educate clients/care providers on treatment strategies.
  • Maintain active OT registration; participate in education activities and Bayshore policies.

Job description

The Occupational Therapist, under the direction of the Director Therapy & Rehab, Manager of Rehab, or designate, is responsible for completion of professional activities that include: assessment, treatment, education, and management of clients requiring Occupational Therapy intervention, in accordance with professional standards.

Clinical Responsibilities
  • Responsible for the assessment of clients, establishment and ongoing evaluation and revision of treatment programs, including goals and discharge plans.
  • Maintains current accurate clinical records according to standards set by College of Occupational Therapists of applicable province. Documents assessment results, goals, treatment, client progress and discharge plans.
  • Collaborates with other members of the health team as needed.
  • Communicates pertinent information and recommendations to clients/care providers and provides education regarding treatment strategies as needed.
Other Responsibilities
  • Maintain active standing in College of Occupational Therapists of applicable province.
  • Participate in ongoing internal and/or external education activities
  • Adhere to Bayshore Therapy & Rehab Policies and Procedures
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System
  • Participate in proactive Health & Safety activities while performing all duties. Responsible to notify immediate Supervisor of any Health & Safety risks or concerns.
  • Maintain confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel
  • Complete other tasks as requested
REPORTING RELATIONSHIPS

The Occupational Therapist reports to the Director Therapy & Rehabilitation, Manager of Rehab or designate as appropriate.

Education

Masters Degree or recognized equivalent in Occupational Therapy. Registered and in good standing with College of Occupational Therapists of Ontario

Experience

Preference of three years providing Occupational Therapy services. Experience with in-home assessments and treatments; knowledge of auto insurance and the legal system, particularly medical/legal report writing is an asset. ADP is an asset.

Enjoy working with clients of all ages in multiple treatment environments. Strong communication (oral and written) and presentation skills. Exceptional interpersonal and decision-making skills. Ability to handle difficult situations in an objective consistent manner. Ability to work independently and effectively collaborate with a team. Demonstrated knowledge of personal computers and proficiency with secure email. Commitment to continual learning. Valid driver's license.

STANDARDS OF PERFORMANCE

The Occupational Therapist must demonstrate ongoing competency in completing all duties and responsibilities of this job description, in response to changes by the regulating body, as well as agreed upon specific goals and objectives.

PERFORMANCE REVIEW

Performance will be reviewed on an ongoing basis with a formal review if required. Input will be sought from employees and consultants at the branch and National Service Centre as applicable.
