Job summary

CBI Home Health is seeking an Occupational Therapist to serve the Durham Region (Whitby/Oshawa) in a home-based Integrated Care Program. The OT will perform functional assessments, implement ADL/IADL strategies, provide equipment recommendations, and collaborate with care teams to support safe hospital-to-home transitions.

You will work within a flexible schedule to meet patient needs, with opportunities across full-time, part-time, and casual arrangements.

Qualifications

• Registered with the College of Occupational Therapists of Ontario in good standing.
• Ability to travel within the Durham area (valid driver’s license and insurance).
• Coordinated, person- and family-centered care approach.
• Strong written and verbal communication, critical thinking, and clinical judgment.
• Able to work in both team settings and independently.

Responsibilities

• Provide OT interventions, including ADL/IADL strategies, equipment recommendations, and home modifications.
• Collaborate with interdisciplinary care teams to deliver integrated home health services.
