Job summary

RhynoCare seeks an experienced Nurse Practitioner to work in Long Term Care facilities in London, Ontario. The NP will be employed as an independent contractor and deliver comprehensive primary health care to residents.

Responsibilities include on-call duties, clinical leadership, and collaboration with a multidisciplinary team to ensure resident well-being, safety, and continuity of care. Commitment to evidence-based practice and quality improvement is required.

Qualifications

• Registration with the College of Nurses of Ontario (Extended Class)
• Bachelor's degree in Nursing; MSN with NP specialization preferred
• Membership with Nurse Practitioners' Association of Ontario (preferred)
• Professional liability insurance (RNAO or other)
• Vulnerable Sector Check valid
• CPR certification required
• Knowledge of RNAO Best Practice guidelines and MLTC Regulations and Act

Responsibilities

• Delivers primary health care to LTC residents with acute and chronic conditions
• Provide on-call coverage as required
• Conduct health assessments and develop person-centered care plans
• Collaborates with residents, families and multidisciplinary teams
• Supports discharge planning and higher levels of care when needed
• Engage in quality improvement, education, and professional advisory activities
