About the Opportunity:
An established long-term care facility in Toronto is seeking a dedicated Nurse Practitioner (NP) to join its clinical leadership team. This is a hands-on, high-impact role focused on delivering primary care to residents while also supporting research, education, and quality improvement initiatives.

What You’ll Be Doing:
Resident Care (70–80%)
- Deliver comprehensive primary care to residents with acute and chronic conditions.
- Conduct detailed health assessments and physical exams; order and interpret diagnostic tests.
- Develop care plans in partnership with residents, families, and team members.
- Manage hospital discharges, palliative and end-of-life care, pain and dementia care.
- Apply knowledge of diverse health conditions and social determinants to support well-rounded care.
- Act as a liaison with hospitals and health partners to ensure continuity of care.
Leadership, Research & Education (20–30%)
- Participate in interdisciplinary research projects and quality improvement initiatives.
- Mentor and educate clinical staff, students, and other healthcare professionals.
- Support the adoption of best practices and compliance with long-term care regulations.
- Represent the care team on internal committees and professional advisory groups.
What You Bring:
- Nurse Practitioner registration with the College of Nurses of Ontario (Extended Class).
- BScN required; MScN or MN preferred.
- Membership with NPAO and liability insurance (e.g., through RNAO) preferred.
- Certification in gerontology is an asset.
- Previous experience in long-term care or geriatric settings strongly preferred.
- Strong clinical reasoning and communication skills.
- A people-first mindset with the ability to navigate complex or high-needs resident populations.
- Flexibility to provide after-hours or on-call coverage as needed.
- This is an independent contractor position – no benefits or paid time off included.
- On-site work required in a central Toronto long-term care facility.
- While full-time is preferred, qualified part-time applicants are welcome to apply.
