Qualifications

• Diploma in health care or related field; RN/RPN/LPN with active provincial registration preferred.
• Knowledge in clinical operations, care coordination, and case management processes.
• Knowledge in patient support program workflows and therapy access pathways.
• Knowledge in clinical documentation, record maintenance, and audit requirements.
• Knowledge in treatment protocols and patient education practices.
• Knowledge in reimbursement navigation and coverage processes.

Responsibilities

• Review patient files, treatment history, and provider inputs to confirm readiness for case action.
• Execute clinical and operational procedures to support enrollment and therapy coordination.
• Audit case activity and documentation to identify discrepancies and delays.
• Evaluate therapy access barriers and determine escalation needs.
• Interpret program protocols to guide patients, providers, and internal teams.
• Coordinate with providers, pharmacies, clinics, and support teams to stabilize the patient journey.
• Uphold adverse event reporting and regulatory documentation standards.
