Job summary

Cencora in Canada is seeking a Nurse Case Manager to support patient and provider access programs within the Innomar Strategies team. You will assess patient needs, coordinate with healthcare providers, and document cases in the electronic patient record to ensure timely therapy access.

The ideal candidate holds a health care diploma with RN/RPN/LPN registration in the province, has 2–5 years of healthcare or insurance experience, and is fluent in French and/or English.

Qualifications

• Diploma in health care or related field (RN/RPN/LPN preferred).
• 2–5 years in healthcare, pharma, insurance or related fields.
• Fluency in French and/or English may be required depending on program and location.

Responsibilities

• Provide patient and provider information for Patient Support services and therapy access.
• Coordinate with healthcare providers and clinics for treatment logistics.
• Document patient information and interactions in the electronic patient record to track journey and outcomes.
• Assist in data collection and follow-up to ensure continuity of care.
• Collaborate with reimbursement specialists and physicians to secure funding for therapies.
