Job summary

Cencora is seeking a bilingual Nurse Case Manager to support patient and provider services in Canada through Innomar Strategies. The role involves coordinating patient care, ensuring high-quality application delivery, and supervising application development activities while maintaining strong stakeholder relationships.

The successful candidate will hold a health care diploma and nursing license, with 2–5 years healthcare experience; French and/or English fluency is required.

Qualifications

• Diploma in health care or related field.
• Licensed Nurse (RN or RPN/LPN) with active registration.
• Minimum of two to five years in healthcare, pharmaceutical, insurance or related fields.

Responsibilities

• Provide support to patients and health care providers by offering information on Patient Support services, diseases, and therapies.
• Assist patients and caregivers in conducting assessments related to Patient Support services.
• Act as a liaison between patients, families and health care providers for medication access.
• Document patient information and interactions within the electronic patient record.
• Follow case-management protocols, data collection, patient follow-up and adverse event reporting.
