Job summary

Autism Services in Saskatoon is seeking a Chronic Disease Management Nurse Educator to join the Allergy & Asthma Clinic on a part-time basis. The role reports to the Chronic Disease Management Manager and focuses on respiratory disease self-management education for patients and families.

Responsibilities include coordinating education programs, liaising with clinicians and community agencies, and contributing to program development and quality improvement initiatives.

Qualifications

• Three (3) years nursing experience with patients/families with chronic diseases.
• Experience in respiratory disease management programs is desirable.

Responsibilities

• Primary responsibility for chronic disease management programs in respiratory diseases.
• Assess and plan self-management care needs for clients; provide education and counseling.
• Coordinate group education classes for outpatients and liaise with families, physicians and community agencies.
• Participate in program development, planning, quality improvement and evaluation.

• Job Description: REPORTING TO THE CHRONIC DISEASE MANAGEMENT MANAGER - RESPIRATORY PROGRAMS, THE RESPONSIBILITIES OF THE CHRONIC DISEASE MANAGEMENT NURSE EDUCATOR WILL INCLUDE: PRIMARY RESPONSIBILITY FOR CHRONIC DISEASE MANAGEMENT PROGRAMS IN THE AREAS OF RESPIRATORY DISEASES - ASTHMA, ASSISTING WITH DEVELOPMENT/CO-ORDINATION/CONDUCT OF INPATIENT AND OUTPATIENT PROGRAMS. UTILIZE ESTABLISHED CRITERIA TO ASSESS AND PLAN EACH CLIENT'S SELF-MANAGEMENT CARE NEEDS, PROVIDING APPROPRIATE INTERVENTION TO ENSURE TIMELY ACCESS TO CARE. PROVIDE EFFICIENT AND EFFECTIVE INDIVIDUAL SELF-MANAGEMENT EDUCATION/COUNSELING FOR PATIENTS/FAMILIES/CAREGIVERS IN ALL ASPECT OF RESPIRATORY DISEASE MANAGEMENT CARE AND TREATMENT. THIS WILL INCLUDE LIAISON WITH FAMILIES, PHYSICIANS, DIETITIANS, STAFF NURSES, AND COMMUNITY AGENCIES. COORDINATE AND FACILITATE GROUP EDUCATION CLASSES FOR OUTPATIENTS IN A VARIETY OF SETTINGS. PROMOTE CONTINUING HEALTH PROFESSIONAL INVOLVEMENT IN PATIENT EDUCATION THROUGH EFFECTIVE COMMUNICATION, DEVELOPMENT OF RESOURCE MATERIALS AND PROVISION OF IN-SERVICE EDUCATION. PARTICIPATE IN DEPARTMENTAL/RESEARCH ACTIVITIES (PROGRAM DEVELOPMENT, PLANNING, QUALITY IMPROVEMENT, AND EVALUATION).
