Job summary

Saskatchewan Health Authority is seeking a CDM Clinical Nurse Educator at Meadow Lake Primary Health Care Centre to lead self-care education for chronic disease management. You will work with an interdisciplinary team to deliver group and individual sessions.

The role requires BScN (or equivalent), 3 years of relevant clinical experience, and CRNS eligibility. Travel is required and CNA or national certifications are assets.

Qualifications

Job Description: The CDM Clinical Nurse Educator reports to the Manager Clinical Integration and works with an interdisciplinary team with a chronic disease prevention and management focus. The CDM Clinical Nurse Educator combines in-depth clinical expertise, with the principles of teaching and learning, in the provision of self-care management for clients, families, health professionals and students in group and individual settings.

Experience

• 3 years clinical experience with patients/families with chronic disease

Education

• Bachelor of Science in Nursing (BScN)

Licenses

• Valid Class 5 driver's license

Other Education and Training

• Basic Life Support Certificate (BLS)
• Nursing diploma is acceptable
