Job summary

The Care Company is seeking compassionate Registered Nurses (RN) and Registered Practical Nurses (RPN) to join our Visiting Nursing Team in Ontario. This mobile role involves traveling between client homes to deliver skilled nursing care, post-hospital support, wound care, and medication management within a defined geographic area.

You will maintain detailed clinical records and collaborate with an interdisciplinary team to ensure high-quality, person-centred care.

Qualifications

  • Active registration and good standing with CNO as RN or RPN.
  • CPR and First Aid certification current.
  • WHMIS certification current.
  • Negative TB test per onboarding.
  • Criminal Record Check with Vulnerable Sector Screening.
  • Ability to communicate effectively in English, verbal and written.
  • Valid driver’s license and vehicle for client-home travel.

We are currently seeking compassionate and clinically confident Registered Nurses (RNs) and Registered Practical Nurses (RPNs) to join our Visiting Nursing Team.

This role is ideal for nurses who enjoy working independently in the community, managing a varied caseload, and providing skilled nursing care to clients in their homes. Rather than being assigned to one client for an extended shift, Visiting Nurses travel within a designated geographic area to provide scheduled nursing visits based on individual client needs.

Geographic areas include Central (Bradford, Simcoe, East Gwillimbury, Georgina, North York, Markham, Thornhill, Richmond Hill, etc.) and Central East Regions of OH@H (Scarborough, Ajax, Whitby, Oshawa, Peterborough, Haliburton, Kawartha Lakes, etc.).

Our Visiting Nurses support clients with a wide range of health needs, including post-hospital care, medication management, wound care, chronic disease management, catheter and ostomy care, enteral feeding, wellness assessments, vital sign monitoring, and other skilled nursing services.

Previous experience providing visiting nursing services in the community and/or supporting Ontario Health atHome (formerly HCCSS) clients is a strong asset.

What You'll Do

As a Visiting Nurse with The Care Company, you will:

  • Complete comprehensive nursing assessments and develop, implement, evaluate, and update individualized plans of care based on client needs, clinical findings, medical orders, and evidence-informed nursing practice.
  • Manage a community caseload and, when designated as the primary provider, act as the point-of-care lead while collaborating with other members of the client's healthcare team.
  • Provide skilled nursing interventions in clients' homes, which may include wound assessment and treatment, medication administration, injections, catheter care, ostomy care, enteral feeding and G-tube care, oxygen therapy, and other nursing procedures within the nurse's scope of practice and demonstrated competency.
  • Administer medications orally, topically, subcutaneously, through enteral routes, or by other authorized routes in accordance with medical orders, CNO standards, and the client's plan of care.
  • Assess and monitor vital signs, pain, skin integrity, respiratory status, cognition, mobility, and overall health status and respond appropriately to abnormal findings or changes in condition.
  • Provide wound care in accordance with prescribed treatment plans, including wound assessment, dressing changes, monitoring healing, and escalating concerns when required.
  • Educate clients and caregivers regarding medications, treatments, disease management, equipment use, symptom monitoring, safety, and strategies for managing care at home.
  • Support clients through care transitions by collaborating with hospitals, physicians, Ontario Health atHome, pharmacies, rehabilitation professionals, community agencies, and other members of the interdisciplinary team.
  • Identify changes in client needs and communicate concerns promptly to the appropriate Care Coordinator, physician, Clinical Supervisor, or other healthcare provider.
  • Maintain accurate and timely electronic documentation, including nursing assessments, progress notes, medication administration, incident reports, and other required clinical records.
  • Practice appropriate infection prevention and control, including the proper use and disposal of PPE.
  • Independently organize and manage assigned visits while communicating proactively with The Care Company regarding scheduling, client concerns, and changes in care needs.
  • Maintain professional, therapeutic relationships with clients and families while delivering compassionate, respectful, and culturally sensitive care.
  • Practice in accordance with College of Nurses of Ontario (CNO) standards, applicable legislation, and The Care Company's policies and procedures
Requirements
What We're Looking For

Our Visiting Nurses must be comfortable working independently in clients' homes while recognizing when additional clinical support or escalation is required.

The following are mandatory:

  • Active registration and good standing with the College of Nurses of Ontario (CNO) as an RN or RPN.
  • Current CPR and First Aid certification through a recognized training provider.
  • Current WHMIS certification.
  • Negative TB test meeting The Care Company's onboarding requirements.
  • Satisfactory Criminal Record Check with Vulnerable Sector Screening.
  • Ability to communicate effectively in English, both verbally and in writing, including completion of professional clinical documentation.
  • Valid driver's licence and reliable access to a vehicle, as travel between client homes is an essential component of the role.
  • Ability and willingness to provide care to clients of varying ages, genders, backgrounds, and care needs.
  • Strong clinical assessment, critical-thinking, problem-solving, and decision-making skills.
  • Ability to recognize, respond to, and appropriately elevate changes in a client's condition.
  • Excellent organizational and time-management skills with the ability to independently manage multiple scheduled visits.
  • Ability to work independently while collaborating effectively with an interdisciplinary healthcare team.
  • Flexibility and adaptability in response to changing client and community needs.
Strong Assets
  • Previous Visiting Nurse experience in home and community care.
  • Previous experience supporting Ontario Health atHome (formerly HCCSS) clients.
  • Experience managing an independent community nursing caseload.
  • Experience with wound care, including complex or chronic wounds.
  • Experience with medication management and injections.
  • Experience with catheter, ostomy, enteral feeding, G-tube, respiratory, or palliative care.
  • Experience supporting clients following hospital discharge or during transitions in care.
  • Gerontology competencies and experience supporting older adults.
  • Experience with electronic clinical documentation systems.
Scheduling & Travel

This is a mobile community nursing position. Nurses will travel between client homes within an assigned or mutually agreed-upon geographic area.

  • Casual and flexible scheduling based on nurse availability and client needs.
  • Opportunities may include daytime, evening, and weekend visits.
  • Caseloads and available visits will vary based on current community referrals.
  • Nurses may identify the geographic areas in which they are available to provide care.
  • Reliable transportation is required to travel efficiently between scheduled client visits.
  • Visits will be scheduled with consideration for geography and travel efficiency wherever possible.

Compensation & Benefits

  • Competitive rates of pay: $33.25-$43.25/hour, depending on nursing designation and applicable assignment.
  • Mileage reimbursement for eligible travel between client visits if 50km is exceed in between client visits. Please note this would not apply from home to first visit and from last visit to home.
