Job summary

AgeCare is seeking an experienced MDS Coordinator to oversee the MDS process for the interdisciplinary team in a long-term care setting. You will supervise MDS documentation, provide training, and ensure timely assessments across the residence.

The role requires RN or RPN licensure in good standing and a solid background in Long-Term Care, with leadership experience preferred. Ontario-based, on-site position offering a competitive salary.

Qualifications

• Long-term care knowledge and experience.
• Familiarity with the Long-Term Care Act and Ministry of Health LTC Program Standards.
• Minimum of three years clinical, project leadership, or managerial experience in Long Term Care.
• RN or RPN in good standing with the College of Nurses of Ontario.

Responsibilities

• Coordinate and implement the MDS for the interdisciplinary team.
• Provide supervision and assistance to the team regarding MDS documentation.
• Deliver MDS education and training to residence staff using train-the-trainer approach.
• Monitor on-site MDS usage and provide on-site support to all users.
• Develop and maintain an up-to-date MDS assessment schedule and ensure timelines are met.
• Audit MDS completion and coding accuracy across the residence.
