Program Overview
The Licensed Practical Nurse –Clinical Integration Nurse as part of the Medical Home Support Program provides comprehensive care to clients by assessing, identifying and facilitating various services in collaboration with other professionals within a multi-disciplinary team. The LPN – Clinical Integration Nurse accepts responsibility for continuing competency through ongoing professional development, including participation in education programs, research and continuous quality activities.
Position
The LPN- Clinical Integration Nurse will work in a physician’s office and participate in the provision of care to patients by applying nursing knowledge, skills and judgment to assess patient needs and provide nursing care in collaboration with Medical Home Support Program team members and community agencies or programs.
