Home Instead is looking for a dynamic and caring person to join our team as a Case Manager LPN for a permanent 5-day week position. An active LPN registration is required. The Case Manager LPN will work closely with families at the beginning of their care journey, providing care solutions for their loved ones, supporting Care Givers, conducting training, and holding family meetings.

Home Instead is a provider of premium home care services to our elderly clients. This role is distinct from working in a clinic or care home setting; our LPN Client Care Coordinators have a broader scope of care than typical clinical roles.

To be successful, the candidate must live in Leduc and:

  • Current LPN registration is required
  • Have a valid driver’s license with a vehicle
  • Be articulate with excellent command of the English language
  • Have at least 2+ years of home care experience
  • Be able to work with a diverse customer base
  • Be capable of multi-tasking in a busy environment
  • Communicate confidently and professionally
  • Sales experience is a plus

This position involves speaking with family members seeking answers about their senior loved one’s care needs. You will interact with a variety of people at different stages of their care journey, requiring confident and strategic communication skills.

You will need to use influencing skills to explain why families should choose Home Instead. The role involves managing multiple client demands that change over time. You must work well in a fast-paced environment, be service-oriented, and collaborate effectively with a team. Your healthcare background will help you understand our clients’ unique challenges. We seek a hardworking individual who thrives in a busy setting.

This is how we do home care! No two days are alike, as you will be visiting clients and their families throughout the city, which is very different from a clinic or facility role.

The Case Manager LPN will perform duties related to client care, including care consultations, client and Care Giver introductions, and quality assurance visits. Using a consultative sales approach, you will assess individual needs, provide tailored solutions, and evaluate care plans through ongoing communication and visits to ensure high-quality care, satisfaction, and opportunities for increased service hours. You will often speak with families uncertain about the best care pathway, so confidence and professionalism are essential.

This leadership role includes teaching and training our unregulated workforce through classroom sessions and in-home visits to support and train Care Givers.

Qualifications:

  • Excellent written and verbal English communication skills
  • Experience working with seniors and their families
  • 5+ years of healthcare experience
  • Experience in sales and customer service
  • Supervisory experience is an asset
