1. Patient Monitoring & Support

1. Conduct virtual check-ins to assess patient status and well-being.
2. Monitor vital signs remotely if the patient uses connected health devices.
3. Track medication adherence and provide reminders.
4. Assess wound healing or skin conditions via video calls.

2. Care Coordination & Documentation

1. Coordinate care between home care aides, nurses, and doctors.
2. Update electronic medical records (EMRs) and maintain patient charts.
3. Schedule appointments and follow-ups with healthcare providers.
4. Communicate with pharmacies for medication refills or changes.

3. Patient & Family Education
