Job Description

In accordance with the Vision, Purpose, and Values, and strategic direction of the Vancouver Island Health Authority (Island Health), patient and staff safety is a priority and a shared responsibility. Continuous quality and safety improvements are essential in this role.

Reporting to the Manager or designated supervisor, the Licensed Practical Nurse (LPN) works independently and within an interdisciplinary team to provide personal care, perform nursing procedures, conduct home visits, and complete assessments to ensure quality client care. The LPN contributes to developing client-centered care and self-care management standards, policies, and procedures. The role includes supporting and directing Community Health Workers and collaborating with healthcare professionals to facilitate quality care.

The LPN operates within the full scope of practice according to the Standards of Practice outlined by the BC College of Nurses and Midwives and adheres to Home and Community Care Program policies, standards, and protocols. Travel may be required, and transportation arrangements must meet Island Health operational requirements, potentially including the use of a personal vehicle.

Qualifications
Education, Training, and Experience
  • Registration with BC College of Nurses and Midwives as a practicing LPN.
  • Additional education as required by BCCNM for practice involving Mental Health and/or Substance Use primary diagnoses.
  • Basic Life Support Level C certification.
  • Two years of recent related experience.
  • Valid BC Driver’s license.
Skills and Abilities
  • Knowledge of and competencies in the full scope of practice as required by the BC College of Nurses and Midwives.
  • Effective verbal and written communication skills.
  • Strong observation and client assessment skills.
  • Ability to organize and prioritize work for oneself and others.
  • Provision of culturally sensitive care and services.
