If you enjoy providing services to clients in a community-based setting, this position will be of interest to you. The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team, utilizing the nursing process to provide direct nursing care to individuals across the lifespan. Working as a Case Manager, the LPN is accountable for assigned caseloads, with the primary responsibility of assessing client needs, determining service requirements, negotiating service options, care planning, and coordinating transitions across care settings. The LPN also performs interventions and restricted activities for clients as required according to client needs. There is also an expectation to be a mobile worker. This role requires travel of up to 100 km per day to care for clients in their homes or rural communities. Possession of a valid driver's license and access to a motor vehicle in good repair for work-related purposes are requirements of this position. You will be responsible for operating a laptop computer and mobile phone and for performing manual handling tasks (lifting, lowering, carrying, pushing, and pulling) as a regular part of this position. For example, lifting approximately 30 pounds in and out of a vehicle on a regular basis and carrying this weight up and down stairs throughout the day is common in this role. This position is based out of Didsbury Home Care and serves the communities of Didsbury, Carstairs, Crossfield, the Cremona area, and surrounding rural communities.

Description:

You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures.
