Alberta Health Services is seeking a Licensed Practical Nurse (LPN) for Brooks Home Care. You will work within an interprofessional team, perform nursing assessments, care planning, and coordinate transitions across care settings, including travel to client homes.

The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. Working as a Case Manager, the LPN is accountable for assigned caseloads with the primary responsibility to assess client needs, determine service needs, negotiate service options, care planning, and coordination of transitions across care settings. The LPN also performs interventions and restricted activities for clients as required according to the client need.There is also an expectation to be a mobile worker. This role requires travel to care for clients in their homes, including rural communities. Possession of a valid drivers’ license and access to a motor vehicle in good repair for work related purposes is a requirement of this position. You will be responsible to operate a laptop computer and mobile phone. Standing, walking, lifting, pushing, pulling, stooping and carrying equipment is required throughout the shift.
As a Licensed Practical Nurse (LPN), you are part of an interprofessional health care team, utilizing nursing processes, critical thinking, problem solving, and decision making skills. You will be guided by your education, experience, and demonstrated skill level, in alignment with applicable legislation, standards of practice, policies, and procedures. You will play a key role in providing safe, high quality, patient and family centred care while reflecting the shared vision and values of the organization.
Completion of an accredited practical nurse education program.Active or eligible for registration and a practice permit as a Licensed Practical Nurse with the College of LPNs and HCAs of Alberta (CLHA).Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
Additional Required Qualifications:Completion of an accredited Licensed Practical Nurse education programActive or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA)Minimum of two years current nursing experience in an Acute Care settingRecent experience working with complex wounds, surgical drains, & IV (HPTP) therapyKnowledge, skill, ability and willingness to work to full scope of practice.Commitment to continuous learning relevant to LPN practiceAbility to utilize critical thinking skills in the practice of conducting initial and ongoing nursing assessments, care planning and service delivery.Ability to utilize critical inquiry to assess, plan and evaluate the effectiveness of interventions on client outcomes.Excellent verbal and written communication skillsBasic use of technology and competency in computer, clinical electronic device use, and electronic documentation.Current Basic Cardiac Life Support – Health Care Provider (BCLS-HCP) from the Heart and Stroke Foundation within the past yearAccess to a vehicle in good repair for work related purposes
Preferred Qualifications:Recent work experience in a community-based setting, Home Care experience preferredExperience working with complex chronic disease(s) and Palliative clients.Experience in Case management practice and completion of the McMaster AHS Case Management educationAdvanced wound care certificate
