Qualifikationen

• Minimum one year verifiable experience working directly with youth with complex behavioural and mental health needs.
• In-depth knowledge of child and youth care mental wellness and home care best practices.
• Flawless execution of documentation requirements and record-keeping.
• Strong written and verbal communication skills.
• Post-secondary degree/diploma in Child and Youth Care or Social Work preferred.

Aufgaben

• Provide daily updates to Program Supervisor and Manager.
• Report critical incidents to Program Supervisor.
• Support youths in planning and achieving goals.
• Maintain activities, observations, and direct hours of service records.
• Compile data for Case Team meetings and stakeholder reporting.
