Ontario Health atHome is seeking an experienced Care Coordinator (Full-Time) in Timmins to assess patient care needs, determine eligibility and coordinate with patients, families and providers for connected, patient-centred care. You will link patients to community services, monitor care plans, and support a busy multidisciplinary team.

As a Care Coordinator (Status: Full-Time, Start date: Immediately; Location: Timmins) you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.
