Job summary

Jobtailor is hiring a Home Health Nurse to coordinate at‑home patient care across Ontario. You will assess needs, plan and monitor care, and collaborate with interdisciplinary teams to ensure high-quality outcomes for diverse clients.

The role requires RN or RPN, valid First Aid/CPR, and a clean Ontario license. Expect a bi‑weekly weekend rotation, strong documentation, and ongoing professional growth.

Qualifications

Job description

• Inform the nursing supervisor of client activities, relevant concerns, or changes in condition or care plan
• Submit appropriate documentation to the case coordinator
• Maintain documentation standards according to the College of Nurses of Ontario
• Work within ISDS to strengthen integrated care, promote interdisciplinary collaboration, and provide a seamless client experience

Requirements

• Registered and in good standing with the College of Nurses of Ontario (CNO)
• RPN diploma or RN diploma/degree from a recognized college or university
• Hold a valid First Aid or CPR Certification
• A valid Ontario driver's license (G License) and a reliable vehicle for traveling are required
• Morning and day availability
• Bi-weekly weekend rotation, from 8:00 a.m.-4:00 p.m. or 9:00 a.m.-5:00 p.m.
• Effective written and verbal communication and documentation skills
• Reliable attendance, punctuality, and availability
• Commitment to lifelong learning and professional growth
• Ability to apply critical thinking skills to provide client care
• Membership of your Professional Association preferred (RNAO/WeRPN)
• Moderate wound care or venipuncture knowledge and experience is an asset
• Acute Care and Medical-Surgical nursing experience is an asset
• Knowledge of community nursing is an asset
