Job summary

London Family Health Team is seeking an Intake Coordinator to support patient attachment and onboarding within Ontario’s IPCT priorities.   • Ability to manage competing priorities and work effectively as part of an interdisciplinary team.

London Family Health Team – Intake Coordinator – Primary Care

London Family Health Team (LFHT) is seeking a highly organized and collaborative Intake Coordinator to support the implementation of patient attachment and initiatives aligned with Ontario’s Primary Care Interprofessional Team (IPCT) priorities.

Term of Employment: Contract (until March 31, 2027)
Location: Onsite – possible multiple FHT sites

The Intake Coordinator plays a key role in welcoming and onboarding new patients into primary care. This position serves as the first point of contact for patients entering the practice and ensures that the intake process is timely, organized, patient-centred and coordinated with the primary care team.

Working closely with physicians, nurses and allied health professionals, the Intake Coordinator supports patients from initial application or referral through to their first primary care appointment and connection with appropriate services.

Key responsibilities include:

NOTE:

This is an administrative/patient-navigation role and does not involve clinical assessment or diagnosis. Clinical concerns identified during intake are escalated to the appropriate regulated health professional.

Contract Terms:

Hours: Up to 40 hours per week (to be negotiated)

Rate: $25.00 – $28.00 per hour
