Job summary

The London Family Health Team in London, Ontario seeks a Nurse Practitioner to provide patient-centered primary care across the lifespan within an integrated, interdisciplinary model. This full-time NP role emphasizes health promotion, disease prevention and chronic disease management.

Qualifications

  • Current Registration with the College of Nurses of Ontario in the Extended Class.
  • Membership with the Registered Nurses Association of Ontario.
  • Master of Nursing – Primary Care NP Program (PHC-NP certificate).
  • Minimum 4 years of most responsible provider NP experience in primary care or community health.
  • Strong assessment, clinical judgment, and communication skills.
  • Proven ability to work effectively within an interdisciplinary team.
  • Computer literacy, including use of EMR – experience with TELUS PS Suites and OceanMD an asset.
  • Demonstrated leadership, critical thinking, organization and communication skills.
  • An interest in program planning and group program delivery.
  • Ability to work extended hours is required, including after hours clinic (possible).

Responsibilities

  • Provide health promotion, disease prevention, acute episodic care and chronic disease management to patients.
  • Enhance access to assessment, diagnosis and treatment within the rostered community and/or target population.
  • Consult with interdisciplinary team to assess, plan, implement, evaluate and coordinate a patient’s plan of care.
  • Coordinate the interdisciplinary plan of care to facilitate efficient movement through the health care system.
  • Provide leadership in the development, implementation and evaluation of quality management programs.
  • Participate and provide leadership on committees.
  • Contribute to development/implementation and evaluation of patient education programs.
  • Conduct educational teaching sessions with individuals, groups of patients and/or families on health promotion and disease prevention.
  • Mentor staff during provision of direct care activities.
  • Serve as educator and clinical preceptor for nursing students in NP programs.
  • Perform other duties consistent with the job classification.

The London Family Health Team (LFHT) is seeking a highly organized, resourceful and detail-oriented Nurse Practitioner to join our team.

The Nurse Practitioner (NP) will work within an integrated, interdisciplinary care model, collaborating with physicians and other healthcare professionals to provide person-centered, evidence-informed primary care across the lifespan.


The NP will provide comprehensive assessment, diagnosis, treatment, management, and follow-up of patients, with a focus on health promotion, disease prevention, chronic disease management, and continuity of care. The NP applies advanced clinical knowledge, critical thinking, and independent clinical judgment to assess and manage patients with complex and evolving health needs.

Position Responsibilities:

Practices in accordance with the College of Nurses of Ontario Practice Standards for Registered Nurse in the Extended Class.

  • Provide primary health care to patients in accordance with standards set by governing body
    and that is aligned with the overall strategic direction of the family health team.
  • Provide health promotion, disease prevention, acute episodic care and chronic disease
    management to patients.
  • Enhance access to assessment, diagnosis and treatment within the rostered community
    and/or target population.
  • Consult as appropriate with members of the interdisciplinary team to assess, plan,
    implement, evaluate and coordinate a patient’s plan of care and treatment.
  • Collect, interpret and analyze data from multiple sources to formulate a medical diagnosis
    within scope of practice.
  • Coordinate the interdisciplinary plan of care for patients by liaising with members of the
    interdisciplinary team to facilitate the efficient movement of the patient through the health
    care system; this includes leading the coordinated care planning process.
  • Provide leadership in the development, implementation and evaluation of quality
    management programs.
  • Participate and provide leadership on committees.
  • Contribute to the development/implementation and evaluation of patient education
    programs.
  • Conduct educational teaching sessions with individuals, groups of patients and/or families with a focus on health promotion, disease prevention and impact of illness experience on patient and family.
  • Facilitate professional development of team members.
  • Mentor staff during the provision of their direct care activities.
  • Serve as an educator and clinical preceptor for nursing students in nurse practitioner programs.
  • Perform other duties consistent with the job classification as required.
Qualifications:
  • Current Registration with the College of Nurses of Ontario in the Extended Class.

Compensation package includes
  • Hourly rate: $56.63-60.096 (based on years of experience)
