Job summary

• ASIST, Motivational Interviewing, and harm-reduction experience are assets.
• Maintain accurate, timely documentation per Enviros policies and standards.
• Experience in not-for-profit or social services is an asset.

Program Overview

Shunda Creek is a wilderness-based addiction treatment program serving young adult males between the ages of 18 and 26.

The program combines addiction treatment, therapeutic relationships, experiential learning, and wilderness-based programming to support participants in developing the skills, insight, and resources necessary for sustained recovery and successful community reintegration.

Position Summary

Reporting to the Shunda Creek Recovery Centre Program Manager, the Intake Coordinator/Aftercare Worker will support both pre-admission and aftercare services for participants and applicants of the Shunda Creek Recovery Centre.

The Intake Coordinator/Aftercare Worker will assess and review treatment applications from young adult males interested in attending the program and will support applicants throughout the intake and admission process. This includes helping applicants understand program expectations and requirements, completing necessary documentation, assessing suitability and readiness for treatment, and supporting the development of an appropriate plan for admission.

The position will also include travel to the Shunda Creek Recovery Centre, with frequency to be determined, to collaborate with Key Workers, therapists, and the Treatment Team. This collaboration will support continuity of care and the development of effective aftercare plans as participants prepare to transition from the program back into the community.

The Intake Coordinator/Aftercare Worker will work closely with the therapists, leadership team, and onsite staff at Shunda Creek to support applicants and participants throughout their treatment journey. Aftercare responsibilities may include assisting participants with housing, financial resources, education, employment, community supports, and other resources necessary to support successful community reintegration.

This position may also support other program duties as assigned.

The Intake Coordinator/Aftercare Worker is expected to follow all Enviros policies and procedures, health and safety protocols, legislative requirements, and applicable professional standards. The position will provide support as required to ensure program practices meet organizational, regulatory, and accreditation standards.

Accountabilities

• The Intake Coordinator/Aftercare Worker will respond to applicants within 1–2 working days of receiving an application or inquiry and will conduct interviews and assessments to determine suitability and fit for the program.
• The Intake Coordinator/Aftercare Worker will ensure applicants have a clear understanding of the program, treatment expectations, requirements, and admission process prior to admission.
• Work collaboratively with the full-time Intake Coordinator and the onsite Shunda Creek clinical and treatment team to support a coordinated intake and admission process.
• Support applicants throughout the application process and ensure all required documentation and information are completed and received to support a successful treatment and recovery plan.
• Collaborate with Shift Supervisors and the Shunda Creek Treatment Team to develop an admission plan and establish an appropriate intake date.
• Communicate regularly with the Leadership Team regarding admissions, participant transitions, discharges, and aftercare planning.
• If an applicant is not accepted into the program, provide appropriate support and assist the individual in accessing alternative community resources and services to promote their safety, stability, and well-being.

AFTERCARE

• Ensure that each participant has an appropriate and individualized aftercare plan prior to completing the program.
• Assist participants with accessing and applying for housing, financial assistance, education, employment, identification, and other community resources as required.
• Support participants in identifying and connecting with appropriate community-based treatment, recovery, health, and social service supports.
• Assist participants in developing practical plans that support continued recovery, stability, independence, and successful community reintegration.
• Collaborate with the Shunda Creek Treatment Team and community service providers to promote continuity of care following discharge.
• Provide follow-up support, as appropriate, to help participants navigate barriers and remain connected to identified aftercare supports.
• Maintain appropriate documentation regarding aftercare planning, referrals, follow-up, and participant outcomes.

Qualifications/Experience

• Bachelor’s degree in Addictions, Nursing, or a related field; or a Bachelor’s degree in Human Services with relevant coursework in Addictions. An equivalent combination of education and experience may be considered.
• Individuals eligible for registration under the Health Professions Act must maintain active registration. Individuals may require authorization or permission under applicable regulations to perform restricted psychosocial interventions and/or restricted activities.
• Minimum of 3 years of relevant experience working in the fields of addictions and/or mental health.
• Demonstrated experience assessing and supporting individuals who have used or are currently using substances.
• Experience supporting individuals through treatment transitions, discharge planning, and/or community reintegration is considered an asset.
• Strong understanding of addiction, recovery, mental health, trauma-informed practice, and community-based supports.
• Assets include Applied Suicide Intervention Skills Training (ASIST), Motivational Interviewing, and experience with harm-reduction approaches related to substance use.
• Maintain accurate, timely, and confidential documentation in accordance with Enviros policies, legislative requirements, and professional standards.
• Maintain effective communication and collaboration with applicants, participants, families, community service providers, treatment professionals, and other relevant stakeholders as appropriate.
• Experience working within the not-for-profit or social services sector is considered an asset.
• Strong interpersonal, communication, organizational, documentation, and collaborative skills.
• Ability to work effectively both independently and as part of a multidisciplinary team.
• Willingness and ability to travel to the Shunda Creek Recovery Centre as required.
