Primary Duties and Responsibilities:
- Support patients and healthcare providers with information on Patient Support services, diseases, and therapies using manufacturer materials and product monographs.
- Assist patients and caregivers in assessments related to Patient Support services.
- Act as a liaison for medication access between patients, families, and healthcare providers.
- Address inquiries about program services, diseases, or products.
- Provide ongoing support for disease or product-related questions to promote patient adherence.
- Refer patients to healthcare providers for clinical questions beyond program scope.
- Document patient interactions in electronic records.
- Review and update patient files regularly to reflect their journey.
- Follow case-management protocols, including data collection, patient follow-up, and adverse event reporting.
- Collaborate with reimbursement specialists and physicians to secure funding and explore alternatives.
- Coordinate and track patient services to ensure expedited access to therapies.
- Participate in specialized training sessions on therapies provided by the pharmaceutical team.
- Attend pharmaceutical meetings, patient meetings, conferences, and collaborate with patient advocacy groups.
- Coordinate with healthcare providers, pharmacies, and clinics for treatment logistics and issue resolution.
- Maintain clear communication with managers, team members, and clients regarding patient case management.
- Assist in training new Nurse Case Managers and provide feedback on protocols and processes.
- Report adverse events following program procedures.
- Perform other duties as assigned, including standby/on-call responsibilities.
Experience and Educational Requirements:
- A healthcare diploma or related field, preferably a Bachelor's degree.
- Active nursing license (RN or RPN/LPN) registered with the College of Nurses in the province of employment.
- 2-5 years of experience in healthcare, pharmaceuticals, insurance, or related fields.
- Experience in medical/hospital settings, insurance, or biopharmaceutical industry preferred.
- Fluency in French and/or English depending on the location.
