Job summary

Community Living Society is seeking a Service Coordinator (Independent Living) to support adults with intellectual and developmental disabilities. You will plan and coordinate learning opportunities, activities and events that build independence and social connections within the community.

You will work under the Manager of Independent Living, liaising with Housing Providers and CLS staff to ensure person-centered supports and successful community integration.

Qualifications

• Prior paid or volunteer experience in planning and facilitating group activities that promote social inclusion, skill development, independence, and community participation.
• First Aid and CPR Certificate.
• Clear criminal record (CLS funded).

Responsibilities

• Actively solicit input from Supported Individuals to determine priorities for learning and activities.
• Attend and/or review meetings to determine priorities.
• Identify and access community opportunities and resources.
• Maintain an inventory of resources and materials for IL Supported Individuals and staff.
• Develop a calendar of learning sessions, activities, and events.
• Organize, facilitate and deliver learning opportunities and events.
• Screen and onboard tenants for CLS Housing Partnership units.
• Maintain contracts with Housing Providers to foster relationships.
• Resolve issues between Supported Individuals and Housing Providers.
• Provide support to Self-Advocates Committee and attendees of learning sessions.
