Job summary

ASK Wellness Society in Kamloops is hiring a Life Skills Worker to provide person-centred supports to adults with developmental disabilities, helping with independent living, housing goals, and community participation.

You will conduct individual case management, collaborate with the Community Living Program Coordinator, document carefully, and promote safety and well-being while supporting clients to thrive in local programs and housing options.

Qualifications

• Bachelor of Social Work, Human Service Diploma or equivalent and/or experience with mental health/addiction and homelessness issues.
• Ability to work as a team member in a case management setting involving multi-agency supports.
• Understands and maintains clientele/worker boundaries.
• Strong working knowledge of community resources.
• Excellent communication, writing, documentation and organizational skills.
• Must pass criminal record check for vulnerable sector and reference screening.

Responsibilities

• Conduct Individual Case Management meetings to ensure compliance with program agreements and wellness plans.
• Liaise with the Community Living Program Coordinator to ensure a clear, consistent plan.
• Provide individualized support to multi-barriered clients needing low-barrier housing and wellness plan adherence.
• Assist participants to maintain suite cleanliness and community integration.
• Support participants to live as independently as possible within the local community.
• Monitor emergencies and complete incident reports; respond to crisis situations in collaboration with team.
