Job summary

ParaMed Home Health Care is seeking an RN visiting nurse to provide in‑home care across the community. You will assess patients, execute and monitor care plans, and build rapport with patients and families in diverse settings.

The role includes travel to patient residences, on‑call coverage, and collaboration with a supportive team to deliver high‑quality care while maintaining patient comfort at home.

Qualifications

• Registration with the CNO as a Registered Nurse in good standing.
• Valid driver’s license and access to a vehicle.
• Experience in delivering community care is an asset.
• Reliability and flexibility.
• Willingness to travel to and from patient residences within a designated area.
• Passion for making a difference in your community.
• Required on‑call coverage, including after hours to ensure patient safety and to meet patient care needs.

Responsibilities

• Assess patients using appropriate assessment tools to identify individualized needs.
• Execute and monitor care plans.
• Engage patients using a collaborative approach by building rapport and trust.
• Maintain and update professional patient records related to encounters and plans.
