Job summary

ParaMed is seeking a Licensed Practical Nurse to perform community visits, delivering safe nursing care and independent clinical practice with RN support as needed.

You will assess patient needs, monitor care plans, and maintain accurate records, while building strong relationships with clients and families across diverse settings. The role emphasizes autonomy, scheduling flexibility, and ongoing professional development.

Qualifications

• Active CLPNA license required.
• Valid driver’s license and access to a vehicle.
• Experience delivering community care is an asset.
• Reliable and flexible with willingness to travel within a designated area.
• On-call coverage, including after hours, to ensure patient safety.

Responsibilities

• Participate in scheduled community visits to meet patient needs.
• Provide ethical and safe nursing care within your scope.
• Assess patients and adjust care plans as required.
• Engage patients to build rapport and trust.
• Maintain and update patient records on encounters and plans.
