Job summary

South Calgary Primary Care Network is seeking a Mental Health Therapist to join a collaborative primary care team in Calgary. You will provide psychosocial assessment, intake, counselling, and evidence-based therapies within a patient-centered Medical Home framework.

You will work in a hybrid model, balancing in-office and remote patient care, and collaborate closely with physicians, social workers, and community partners to deliver timely access to services and improve health outcomes.

Qualifications

• Masters degree in Psychology, Counselling, or Social Work.
• Registered with CAP or ACSW or equivalent
• 3–5 years of experience providing individual, family and/or group therapy

Responsibilities

• Provide individual, couple and group therapy within a primary care interprofessional team.
• Deliver time-limited, evidence-based therapy with patient-centered goals.
• Contribute to group therapy and psychoeducational sessions as needed.
• Coordinate care with physicians, nurses, and community partners.
