Job summary

South Calgary Primary Care Network is seeking a Mental Health Therapist to join a collaborative primary care team in Calgary. You will provide psychosocial assessment, intake, counselling, and evidence-based therapies within a patient-centered Medical Home framework.

You will work in a hybrid model, balancing in-office and remote patient care, and collaborate closely with physicians, social workers, and community partners to deliver timely access to services and improve health outcomes.

Qualifications

• Masters degree in Psychology, Counselling, or Social Work.
• Registered with CAP or ACSW or equivalent
• 3–5 years of experience providing individual, family and/or group therapy

Position Summary

Together with SCPCN and external Medical Home teams, the Mental Health Therapist contributes to a coordinated system of supports and services that delivers high-quality, evidence-based, patient-centred care. As an integral member of the interprofessional primary care team, the Therapist provides psychosocial assessment, intake, consultation, and counselling services in accordance with professional standards of practice and evidence-based treatment models. Working collaboratively within the Patient's Medical Home and a stepped care framework, the Therapist partners with patients, families, providers, and community resources to deliver coordinated, inclusive, and culturally responsive care that supports timely access to appropriate services and positive health outcomes

What Does the Role Do?

Therapy

• Conduct assessment, treatment planning, intervention, and follow-up in accordance with evidence-based practice, professional standards, and the unique needs, strengths, and goals of each patient
• Provide time limited, evidence-based, patient-centred therapy to individuals and couples experiencing psychological, emotional, behavioural or life challenges that require a level of treatment intensity beyond the scope of services typically provided through Primary Care Behavioural Health services within the Medical Home.
• Create and maintain a safe, professional, and therapeutic environment that is patient-centered, goal-oriented and driven by patient-led therapeutic goals.
• Ensure clinical theory supports evidence-informed treatment recommendations.
• Utilize ongoing patient feedback and patient reported outcome measures to enhance treatment effectiveness, and inform clinical decision‑making.
• Collaborate with internal and external Medical Home providers, community partners, and interdisciplinary team members to support coordinated, comprehensive care and positive patient outcomes.
• Facilitate appropriate referrals, transitions, and consultation with specialized mental health, health, social, and community-based services to ensure patients receive the level of care and support required to meet their needs.

Group Facilitation

• Contributes clinical expertise to the identification of patient and population needs and collaborates with interdisciplinary team members, Medical Home providers, and community partners to inform the planning, development, implementation, and evaluation of evidence-based group therapy services and related therapeutic programming.
• Facilitates group therapy interventions, as appropriate, in accordance with evidence-based practice, professional standards, and identified patient needs.
• Provides occasional facilitation of psychoeducational workshops and educational sessions to support patient, provider, and community learning needs.
• Utilizes effective group facilitation strategies to foster participant engagement and maintain a safe, respectful, and therapeutic group environment.
• Provides education and consultation to physicians, healthcare providers, community partners, and other stakeholders, as required.

Generalduties

• Support a collaborative working relationship with family physicians and the interprofessional team, through regular case consultation, care coordination, and shared implementation of patient-centered interventions.
• Participate in relevant professional development activities, regular individual supervision sessions, group supervision and team meetings with , Clinical Lead, Clinical Supervisor and Program Manager
• Collaborateswith the Program Manager, and Clinical services team toidentifyand develop unique approaches to address patient needs and maximize patient care outcomes.
• With support from the Clinical Lead and Clinical Supervisor, actively contributes to equitable and timely access to care by proactively managing caseloads, monitoring service demand and capacity, and supporting efficient patient flow across services.
• Supports the orientation of new members and students under the direction of the Clinical Supervisor.
• Supervises practicum students when capacity permits
• Actively participate in meetings, ongoing program development, evaluation and quality improvement opportunities.
• Actsas a resource for other staff, asappropriate.

Who Should Apply?

• Masters degree in Psychology, Counselling, or Social Work.
• Registered and in good standing with either the College of Alberta Psychologists (CAP) or the Alberta College of Social Workers (with restricted psychosocial intervention authorization or completion of restricted psychosocial intervention application with the ACSW within 6 months of employment).
• Registered Provisional Psychologists are required to have an external CAP supervisor prior to their start date, completion of the EPPP examination within 6 months of employment, and must receive Registered Psychologist status with CAP within one year of employment.
• Preferred minimum of 3-5 years' experience providing individual, family, and/or group therapy to adults, adolescents, and children.
• Experience with remote and in-person service delivery.
• Experience facilitating individual, couple and group sessions.
• Experience in primary care and/or knowledge of primary health care reform and medical home implementation preferred.

Working Conditions

• Normal office environment.
• We offer a hybrid work environment where staff are welcome to work on-site full-time, with flexibility to work from home on agreed upon days. Staff are required to be on-site 5 days per week during their probationary period.
• Evening hours required 1-2 times a week.
• Valid driver’s license with access to a reliable vehicle as minimal travel is required.
• Two supervisory references required.
• Criminal and vulnerable sector background check required.
