Hamilton Family Health Team is recruiting a Social Worker/Mental Health Counsellor to join our interprofessional primary care team in Hamilton, Ontario. The role combines social work and mental health counselling across ages, with options for hybrid in-office and remote care to support accessible services.

The position emphasizes collaboration with family physicians, evidence-based treatments, and engagement with social determinants of health within a diverse community.

Qualifications

• Formal training and experience in evidence-based treatments such as CBT, DBT, ACT, EFFT.
• Post graduate studies in social work or related health field and professional college membership.
• Criminal Records Check/Vulnerable Sector screening and Covid-19 immunization.

Responsibilities

• Conduct biopsychosocial assessments.
• Deliver evidence-based, patient-centered short-term interventions.
• Develop care plans with patients and interprofessional team members.
• Facilitate referrals to community and health services as needed.
• Provide case management and health promotion support.
• Engage in quality improvement and program development.
