Job summary

Horizon Health Network is seeking a Human Service Counselor to assess, support and assist clients in community or hospital settings through therapeutic and rehabilitative programs.

The role includes developing and implementing client support plans, observing changes, and coordinating with a multidisciplinary team to modify services, provide crisis intervention, teach life skills, and help clients access community resources.

Qualifications

• High school graduation plus a human services program/course of six months to one year.
• Community-based Addictions and Mental Health roles require 1 year related experience.
• Must possess a valid license.

Responsibilities

• Participating in the assessment, development and implementation of the client support services plan.
• Observing, documenting and reporting changes in the client (physical, behavioral, psychological).
• Reviewing, evaluating and modifying individual service plans with the multidisciplinary team.
• Intervening in crisis situations and providing emotional support to clients.
• Assisting clients with activities of daily living by teaching lifestyle skills such as parenting and financial planning.
• Transporting and accompanying clients to activities and appointments related to their care.
• Organizing and implementing individual and group therapeutic programs.
• Assisting clients to access services of various government and community agencies.
• Showing others how to perform tasks or duties as part of departmental orientation.
