Job summary

Horizon Health Network in Moncton, NB seeks a Human Service Counselor to assess, support and assist clients within community or hospital settings through therapeutic and rehabilitative programs.

The role involves crisis intervention, client support planning, documenting changes, and collaboration with a multidisciplinary team to help clients access services and develop lifestyle skills.

Qualifications

• High school graduation plus an additional human services program/course of over six months and up to one year or equivalent.
• For community based positions in Addictions and Mental Health must have 1 year of related experience.
• Must possess a valid license.
• Ability to attend work on a regular basis.
• Ability to perform the duties of the position.
• Good work record.

Responsibilities

• Participating in the assessment, development and implementation of the client support services plan.
• Observing, documenting and reporting, physical, behavioral and psychological changes in the client.
• Reviewing, evaluating and modifying individual service plans as part of the multidisciplinary team.
• Intervening in crisis situations and providing emotional support to clients.
• Assisting clients with activities of daily living by teaching and counseling on lifestyle skills such as parenting, financial planning.
• Transporting and accompanying clients to activities and/or appointments related to their care.
• Organizing and implementing individual and group therapeutic programs
• Assisting clients to access services of various government and community agencies.
• Showing others how to perform tasks or duties as part of the departmental orientation.
