Job summary

House of Hope is seeking a Community Reintegration Worker to support clients as they reintegrate into the community. The role involves frontline advocacy, counselling, crisis intervention and coordinating referrals to external resources.

The CRW works under supervision, maintains client records, manages a caseload and writes monthly progress summaries while contributing to agency projects and promoting a positive image in the community.

Qualifications

  • Post-secondary diploma or degree in Social Sciences or Social Services.
  • Minimum one year of frontline work experience in community outreach.
  • Case management experience is an asset.

Responsibilities

  • Provide frontline advocacy, short-term counselling and crisis intervention for clients.
  • Provide clients with referral services to external community resources when appropriate.
  • Administer frontline service functions including keeping up-to-date client records, managing a caseload, writing monthly progress summaries, and monitoring PBC conditions.
  • Provide administrative support to the agency as tasked by the Executive Director or designate.
  • Engage in special projects and promote goodwill and a positive image of the agency with all members of the public.
