Job summary

Qualifications

• Degree or diploma in social services or equivalent.
• Crisis intervention, de-escalation, counseling and advocacy skills.
• Excellent customer service, organizational and interpersonal skills.
• Strong written and verbal English communication; other languages an asset.

Responsibilities

• Transition to Housing and support clients to maintain housing stability.
• Coordinate housing preparation plans and connect clients to community services.
• Provide referrals and warm transfers to appropriate services as needed.
• Advocate for clients with landlords and social service providers.
• Arrange home visits, monitor client progress and document outcomes.
• Develop community-building activities and arrange for external programs.
• Maintain client files and provide information for funder reports.
