Job summary

Christie Ossington Neighbourhood Centre is seeking a Unionized Housing Follow-Up Support Worker to provide on-site support for residents at 1011 Lansdowne Ave. You will perform intake, assess needs, and deliver housing-focused case management with a harm reduction approach.

The role requires crisis intervention, life skills coaching, and connection to health and social services, while maintaining detailed records and collaborating with landlords and service providers for housing stability.

Qualifications

• College diploma or university degree in social work, community mental health, or a related field.
• Minimum 2 years of direct support experience with vulnerable populations.
• Training in de-escalation and non-violent crisis intervention (e.g., CPI).
• Knowledge of harm reduction, trauma-informed care, case management, and system navigation.
• Understanding of homelessness, mental health, addiction, and housing systems.
• Familiarity with Toronto community resources.
• Strong communication, time management, and organizational skills.
• Problem-solving, conflict resolution, and teamwork.
• Experience with Indigenous communities is an asset.
• Experience in supportive housing or community mental health is an asset.
• Proficiency in MS Office and electronic documentation.
• Current criminal record check.

Responsibilities

• Complete client intake and initial assessments, including documentation and explanation of services.
• Deliver on-site, housing-focused case management to residents using a harm reduction approach.
• Conduct regular on-site visits to support housing stability and prevent returns to homelessness.
• Provide life skills support (e.g., budgeting, tenant rights, housekeeping, pet care).
• Offer supportive counseling, crisis intervention, and system navigation.
• Refer and connect clients to services including healthcare, addictions support, employment programs, and community resources.
• Advocate on behalf of clients with landlords, OW, ODSP, and other service providers.
• Build and maintain positive relationships with landlords to support housing retention.
• Maintain accurate, up-to-date records including case notes and care plans.
• Prioritize safety during client interactions and in service delivery planning.
• Practice trauma-informed, client-centered, and culturally respectful care.
• Participate in case reviews, team meetings, and agency initiatives.
• Contribute to a positive team culture and uphold CONC values.

Christie Ossington Neighbourhood Centre (CONC) is a multi-service community organization supporting individuals and families through shelter, housing, food security, and community-based programs across Toronto. Position Profile

This position is assigned to a housing program located at 1011 Lansdowne Avenue. Unlike some Follow-Up Housing Support Worker positions that operate on a community-based model, this role is primarily site-based and provides on-site support to residents living within the building.

The successful candidate will work closely with tenants who have experienced homelessness and may present with complex and high-acuity needs. The role requires on-site engagement, crisis intervention, housing stabilization support, service coordination, and tenancy support to help residents maintain successful housing outcomes.

This role is designed to promote long-term housing stability by offering individualized support in the community, including crisis intervention, harm reduction, life skills training, and connection to key health and social services.

You will work with individuals facing complex barriers such as mental health challenges, trauma, substance use, immigration issues, and experiences within the criminal justice system. The ideal candidate is compassionate, proactive, and committed to empowering individuals toward greater independence.

This is a unionized position reporting to the Supervisor of Housing Follow-Up Support.
