Southlake Health is seeking a compassionate Social Worker to join the interprofessional healthcare team in Newmarket, ON. You will conduct psychosocial assessments, provide counselling, and advocate for patients and families to support well-being during illness and hospitalization.

The role emphasizes discharge planning, resource linking, and collaboration with physicians and nurses to ensure patient-centered care. Ontario licensure and grief and loss training are required.

Competenze

• Master's degree in social work from an accredited university
• Member in good standing with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
• Membership in the Ontario Association of Social Workers (OASW) preferred
• 3-5 years of relevant social work experience in an acute care or hospital setting
• Experience in oncology, palliative care, or crisis intervention is an asset
• Training or experience in grief and loss required
• Strong knowledge of psychosocial assessment, counselling, and conflict resolution

Mansioni

• Deliver psychosocial assessments and provide counselling, emotional support, crisis intervention, and grief support to patients and families
• Support patients and caregivers in coping with illness and treatment, including family, financial, and social challenges
• Facilitate discharge planning and connect patients and families with appropriate community resources and services
• Collaborate with physicians, nurses, and other interprofessional team members to ensure coordinated, patient-centered care
• Advocate on behalf of patients and families within the healthcare system and community
• Provide consultation and education to healthcare teams regarding psychosocial issues and available resources
• Maintain accurate documentation and statistics in compliance with hospital standards
• Participate in program development, hospital committees, and quality improvement initiatives
