Job summary

Humber River Health in Toronto is seeking a Social Worker to provide psychosocial assessment, crisis intervention, discharge planning and counselling to patients and families across acute and community settings.

You will liaise with patients, families and staff, manage caseload, participate in reflective practice, supervise social work students, and provide cross-coverage across sites including RCC as needed. This is a Temporary Full Time position at the Wilson Site.

Qualifications

• MSW required and current membership with professional bodies is required or eligible.
• Experience in acute/sub-acute settings is asset.
• Computer skills and ability to meet submission timelines.

Responsibilities

• Provide psychosocial assessment, crisis intervention, discharge planning and counselling.
• Act as liaison with patients, families and community resources.
• Discharge planning and patient flow within acute and community sectors.
• Clinical supervision of social work students and cross-coverage across sites.
