St. Joseph’s Health System seeks a Registered Social Worker in Social Work Services to provide psychosocial assessment, counselling, case management and discharge planning across multiple sites in Hamilton. The role emphasizes leadership in high-risk situations, community linkages and ongoing quality improvement.

The incumbent will mentor colleagues, coordinate complex discharges, and ensure care aligns with OCWSSW standards while supporting best practices in patient recovery and quality of life.

• Documents patient interventions per standards and guidelines.
• Conducts comprehensive psychosocial assessments.
• Identifies and intervenes with high-risk social situations.
• Provides theory-based counselling and case management.
• Coordinates complex discharges and leads discharge planning.
• Mentors junior staff and contributes to re-certification activities.
