Job summary

Niagara Health is seeking a Pharmacist for a 1.0 FTE role at Marotta Family Hospital. You will apply clinical knowledge to support safe and effective medication-related care, collaborating with the interprofessional team to optimize therapies.

The position emphasizes patient-centered care, medication reconciliation, and participation in quality improvement initiatives within a hospital setting.

Qualifications

• BSc Pharmacy or PharmD degree.
• Current registration with the Ontario College of Pharmacists in part A of the register, in good standing.
• Hospital pharmacy residency required; or related clinical experience considered.
• Demonstrated ability to prioritize patient care responsibilities.
• Flexible, self-directed and able to work effectively with minimal supervision.

Responsibilities

• Identify and address patient-specific drug-related problems.
• Collaborate with prescribers and the interprofessional team for medication optimization.
• Clinical review and verification of prescriber orders.
• Support the Medication Reconciliation process; engage in quality/safety initiatives.
• Act as a preceptor to pharmacist students and interns.
