Job summary

Niagara Health in Niagara Falls is seeking a Pharmacist for a full-time position at the Niagara Falls site. The role is 0.8 FTE with a rotating schedule including evenings, weekends and statutory holidays.

You will apply clinical judgement to support safe medication-related care, collaborate with prescribers and the interprofessional team, and contribute to quality/safety initiatives within the pharmacy program.

Qualifications

• BSc Pharmacy or PharmD degree.
• Current registration with the Ontario College of Pharmacists in good standing (Part A).
• Hospital pharmacy residency required or equivalent clinical experience.

Responsibilities

• Identify and address patient-specific drug-related problems and optimize therapy.
• Collaborate with prescribers and interprofessional team to optimize medications.
• Conduct clinical review and verification of prescriber orders; support Medication Reconciliation.
• Precept pharmacist students and interns; work within hospital pharmacy practice model.
