Job summary

Carefor Health & Community Services is seeking an experienced Registered Nurse to provide and manage cost‑effective nursing care within a hospice setting in Eastern Ontario. The RN will lead clinical care, teach and delegate to RPNs and other providers, and ensure high‑quality, client‑centred services.

Qualifications

• Certificate of Competence/License as a Registered Nurse in Ontario.
• Vehicle access is a condition of employment for specific Full‑Time positions
• Certificate in Palliative Care (preferred).
• Clinical practice experience (mandatory).
• Experience in Palliative Care (mandatory).
• Experience in Community Health (preferred).
• Strong assessment, risk identification, and risk management skills.
• Skills in collaboration, consultation, and counselling.
• Enhanced chronic disease monitoring and management skills.
• Strong health teaching abilities.
• Excellent verbal and written communication skills.
• Strong interpersonal skills and ability to lead a multidisciplinary team.
• Ability to work effectively with regulated and unregulated care providers and volunteers.
• Strong commitment to and empathy for palliative clients, with an understanding of end‑of‑life care.

Responsibilities

• Provide complete and relevant assessments of clients and families using established procedures.
• Develop individualized care plans in collaboration with clients and families.
• Incorporate discharge planning into the care plan.
• Implement nursing interventions according to the established care plan.
• Perform nursing tasks safely and in alignment with carefor policies.
• Evaluate and document client and family responses to care using established documentation tools.
• Communicate client status regularly with the interdisciplinary team.
• Refer clients to other nursing providers and disciplines as needed.
• Document rationale for referrals, changes in visit frequency, and discharge decisions.
• Provide cost‑effective, organized, outcome‑focused care.
• Demonstrate time management and planning.
• Delegate care to family members and other providers.
• Promote client‑directed care and independence.
• Act as a clinical resource and mentor for staff and students.
